Personal data processing consent
– natural persons above 18 –
I, the undersigned ___________________________, with the phone number ___________________ and the e-mail address ____________________________ By proceeding to access and navigate through this website, hereby expressly and unequivocally agree with the processing of my personal data by data controllers, for the purpose of carrying out the project Interinstitutional cooperation and advocacy for the protection of children victims of sexual abuse and violence, conducted by Save the Children Romania and the General Child Welfare and Protection Directorate of district 6, as part of the “Active Citizens Fund Romania” program, funded using EEA and Norway Grants 2014 – 2021 and managed by CSDF, FP, CRCR and Pact Foundation.
I also hereby declare that I was made aware of the following:
Data that may be subject to processing:
Simple personal data: name, surname, workplace, citizenship, gender, age, phone number, e-mail address;
Purpose of the processing: to carry out and promote the above-mentioned project, to follow-up to check whether its objectives have been met, to implement, manage and follow up on the non-reimbursable funding agreement, to report results and submit explanatory documentation for the achievement of the subject of the non-reimbursable funding agreement, to conduct quantitative and qualitative studies with a view to assessing project outcomes and impact.
Grounds for the processing: The grounds underlying the processing of simple personal is the legitimate interest of data controllers to check whether the objectives for which the personal data were processed have been met – art. 6, para. (1), letter f of Regulation no. 679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data.
The transfer of personal data: these personal data may be transferred between data controllers and third parties within the European Economic Area, particularly across the territory of Romania, Belgium, Norway. Furthermore, these personal data may be transferred to the following categories of persons: funder (Financial Mechanism Office of EEA and Norway Grants), reviewers and/or auditors during inspections they conduct at the request of the funder, external partner within the ACF Romania – Frivillighet Norge (Norway) consortium, fiscal bodies and other public authorities in case of a potential inspection to be carried out by them. The duration for the processing of personal data: 10 years since the final report is approved by the Active Citizens Fund Romania Fund Operator.
Rights concerning the personal data processing:
The right to withdraw consent: I am entitled to withdraw my consent given in this statement for confidential personal data processing, at any moment, provided that the withdrawal of my consent does not impact the legality of the data processing conducted prior to the withdrawal of the consent;
Right to access: I have the right to request data controllers for information as to the processing of my personal data by them.
Right to rectify: I have the right to request data controllers to rectify incorrect personal data.
Right to erasure: I have the right to request data controllers to erase my personal data under specific circumstances, such as: my personal data is no longer necessary to fulfil the purpose of the processing for which it was collected, consent for the processing of these data by controllers has been withdrawn etc.
The right to restriction of processing: I have the right to request data controllers to restrict the processing under specific circumstances, such as when I question the accuracy of the personal data processed or the processing of the personal data is no longer conducted pursuant with the law.
The right to object: I have the right to object to the processing of personal data, at any given moment, for compelling and legitimate reasons related to my particular situation, unless otherwise provided by the law.
The right to data portability: I have the right to request data controllers to send the processed personal data to me or to another entity which will further process my personal data.
Use of the image: I hereby award CSDF, FP, CRCR and Pact Foundation with the right to reproduce, present, use and distribute, indefinitely and not limited to a territory, in any format, my image and my voice photo and/or video and/or audio recorded. I understand and I agree to receive no financial compensation or of any other kind for these photographs/videos or recordings.
